    Waiver

    RELEASE, INDEMNITY, AUTHORIZATION AND ACKNOWLEDGEMENT FOR PARTICIPANT TEAM & PLAYER ENTRY
     
    In consideration of the Cherry Beach Soccer Club ("CBSC") accepting the applicant ("Participating Team" & “Player”) to participate in the Admiral Invitational Soccer Showcase & Festival Sporting Event ("Admiral Event"), the Team and/or Players (Players under 18, the parents, guardians and officials of the team and player need to be aware and recognize the following herin,
     
    1.     Herein acknowledges the risks of injury inherent to the Team/Player’s participation in the sport of soccer; and
    2.     Releases CBSC & Admiral Sporting Event, its directors, officers, representatives, coaches, agents and facility owners/operators from any and all claims or causes of action, for injury, loss or other related expense, howsoever caused, which the Team, Player and/or Spectators may suffer or incur as a result of the Team and/or Player’s participation in Admiral Event; and
    3.     Agrees that at all times during the Admiral Event "Games", participating Teams, and all Players shall wear appropriate shin guards and soccer shoes (cleated for games on grass) as well as the complete uniform or training apparel provided by respective teams, the Player will not wear any jewelry including pierced earrings, Player will only wear prescription glasses when needed; and
    4.     Authorizes and provides consent to CBSC Admiral Event Officials & Organizers to seek medical treatment which in its judgment deems necessary, and further authorizes and consents on behalf of the Player permitting any qualified person or hospital to provide such medical treatment as may be deemed advisable and necessary in the circumstances; and
    5.     Acknowledges all Participating Teams and "Players" maintain medical insurance coverage through Ontario Soccer and/or other foreign FIFA sanctioned policy on all registered Players, and undertakes to immediately advise CBSC Admiral Event Organizers in writing of any claimable injuries occurring during participation in this Admiral Sporting Event, and acknowledges that failure to do so may void coverage; and 
    6.     Authoizes and gives permission to CBSC Admiral Event organizers and officials that at any time may request and demand that Clubs/Academy Teams provide all pertinent documentation at the start, during and/or after Admiral Sporting Event.  This shall inclucde and documentation from Ontario Soccer in regards to Ontario Soccer or other soccer body in the way of original/duplicate Travel Permits, Player and/or Coaches Books, Team Photo Rosters and/or Players/Coaches Cards, Game Sheets and/or other required documentation as required and that every team agrees to provide a Birth Certificate to verify someone's eligibility if requested; and
     7.     Every Participating Team and Player agrees to abide by the published rules of Canada Soccer, Ontario Soccer and those of the CBSC Admiral Event in Exhibition Showcase and/or Festival fomats in which the Team and /or Player participates; and
     8.     Acknowledges that failure to meet entry of enough teams, we may only be able to organize Exhition games and/or issue partial refund and/or in some cases have to withdraw team entry; and 
     9.     Participating Teams, Officials and Players have read and accept the applicable Admiral Sporting Event Showcase & Festival refund policy as outlined in the Admiral Event Rulers & Regulations; and
     10.   Participating Teams agree and consents to the collection, storage and use by CBSC Admiral Event and its agents of personal information of  Teams for future sporting events in facilitating communication between host and participants: and
     11.   Participating Teams, Officials and Players along with Parents/Guardians and respecitve Team Spectators consent to the publication of Club related Admiral Event communications through any media to promote this sporting event; and 
     12.   Agrees to abide by CBSC Admiral Event Rules & Guidelines and standard Code of Conduct as outlined by Players, Officials and Coaches as members of FIFA, Canada Soccer, Ontario Soccer and our own Toronto Soccer Association District. 
     
    For all Players and for Parents/Guardians and/for Team Officials of Players under 18:
    -         I / we will not engage in any kind of unsportsmanlike conduct with any official, coach, parent or player, such as booing, taunting or using profane language or gestures.
    -         I / we will respect the authority of referees; will never criticize, contradict or interfere with referees or coaches at the field; and will speak with coaches privately regarding any concerns.  
    -         I / we will demonstrate and encourage good sportsmanship by showing respect and courtesy, and by expressing positive support for all players, coaches and officials.
    -         I / we will adhere to the policies of all facilities including: water only (no other beverage or food) on artificial turf; no pets anywhere at schools or other fields that do not allow them; no parking in prohibited areas.
    -         I / we also agree that if we or our guests fail to abide by the Regulations, we may be subject to disciplinary action which could include a ban on supporters from attending games, or expulsion of the Player.
     
    Additionally, for Parents/Guardians and for Team Officials of Players under 18:
    -         I / we will remember that youth soccer programs are for the players, not for parents and other adults.
    -         I / we will teach children to play by the rules, and to resolve conflicts without resorting to hostility.
    -         I / we acknowledge that, in children’s leagues, team coaches and club officials are not responsible for the supervision of any child; I/we will supervise any child young enough to normally require adult supervision.
     
    THE TERMS AND CONDITIONS HEREIN, OF THIS RELEASE, INDEMNITY, AUTHORIZATION, CONSENT AND ACKNOWLEDGMENT MUST BE ACCEPTED IN ORDER TO QUALIFY THE APPLICANT TO PARTICIPATE IN THE RESPECTIVE ACTIVITIES OF THE CHERRY BEACH SOCCER CLUB.

    CBSC Codes of Conduct for Players, Officials, Parents, Guardians and Spectators Acknowledgement

    The Respect-in-Soccer Code of Conduct is designed to ensure that soccer can be enjoyed in a safe and positive environment. The codes of conduct noted in attached are an integral part of the very basis of club policy.     Concussion Awareness (Rowan’s Law) Code of Conduct Acknowledgement

    The goal of Rowan’s Law is to protect amateur athletes in any of the by improving concussion safety on the field of play to help prevent similar injuries from happening in the future.     Cherry Beach Soccer Club ("CBSC") respects your privacy and personal data. The Club and Admiral Invitational Showcase/Festival ("Admiral Event") may use any child/children's (players) likeness, voice and/or words in television, radio, film, newsletters, magazines, and other media: and in any form not heretofore described to promote or communicate activities of the CBSC relating to the Showcase in April & Festival in May.     Refund Policy

    Cancellation Policy
    The Admiral Invitational Soccer Showcase/Festival reserves the right to cancel programs when there are not enough team registrants.  Where one is unable to host due to any external factors, please refer to Event Terms & Conditions for details on a full or partial refund. Where a team/official, parent/player, guardian and/or other supporter becomes a danger to the Admiral Event, its members and other members (players, parents, coaches or other support staff) physically or verbally and/or threaten or jeopardize the good name and reputation of Club and/or Event, they will be asked to leave the Club and/or Event.  It will be at the discretion of the Club and/or hosts to determine whether a full, partial or no refund will be issued.  
     
    Refund Policy
    The CBSC Admiral Event agrees to give a full refund when such a request is received in writing via mail and/or email 1 month before the commencement of the Admiral Sporting Event. After this date and before the beginning of the program in question, only a partial refund may be issued at the discretion of Admiral Event Organizers and Officials. However, upon commencement of the actual program/event. NO refund will be issued! It’s at the Club’s discretion if a credit and/or any refund will be issued.  If there are any credits to be issued, there will be minimum admin. fee applied for canceling.  Please understand that there are significant, non-recoverable resources and costs used in providing for the inclusion of a team player in any of the Sporting Event.
